All of these forms are either available online or as downloads.  The downloadable forms are in .pdf format and you will need to use Adobe Acrobat Reader to read and print these forms on your printer.  If you don’t have this software loaded on your PC you can get it for free from Adobe. Follow their instructions carefully to download, install and configure your browser.  Once your browser is configured, just click onto the “download” link next to your desired form. 

Keep in mind where you save the form so you can retrieve it later at your convenience.

Consent Form

For use by the parents and guardians of students enrolled in the eighth through the twelfth grades in Louisiana public schools. The parent or guardian must give consent or deny consent to the collection of their child’s Personally Identifiable Information (PII) and the distribution of the PII through the Louisiana Department of Education to LOSFA and through the Board of Regents to the postsecondary institutions. This form is not required for students completing BESE-approved home-study programs or private school students. The completed form must be submitted to the student’s high school. Download Here

Affidavit of Residency – Hurricane For students enrolled in an eligible Louisiana high school in a designated parish on the designated dates, but who were not enrolled in an eligible Louisiana high school during ALL of the twelfth grade. Download Here
